Terms & Conditions – LIFE Health Packages (GP Clinics & Medical Centres)

Eligibility:

• Above 18 and less than 65 years to avail this package
• It can not be done during menstrual cycle (the Period).
• There is no Physical contact with partner 4-5 Days for the pap Smear report
• Package cannot be avail for pregnant women.

Fasting Requirement
Fasting for 8-10 hours is mandatory. You may drink water but NO solid food.

Consultation Timing:

There will be two visits: the first visit where patients have to provide samples for urine and blood tests, and then the second visit where they can book an appointment within 7 days from registration for the remaining tests and consultation.

Treatment:

·      Any treatment arising from package findings will be applicable only as per Doctor's discretion and may be chargeable.

For packages with Free Consultation:

• Not available in GP Clinics
• Wellness health checkup packages may not serve under any acute illness.
• All packages bought are non-refundable.

Only Gync consultation is inculded

·   If a package does not include any doctor consultation services, the consultation option will not be applicable to that package.

WOW Package:

·      This package is not for Maternity Consultation & Treatment.

·      This is only a screening tool and may not serve as a detailed scan.
